Overview

Summary

 

The Health Transformation Special Projects Supervisor acts as a HealthConnect One (HCO) service facilitator, leader, and coordinator. This role focuses on collaboration, innovation, and specializes in providing patient centered care. They will play a vital role in streamlining processes, identifying areas for improvement, and ensuring projects align with strategic objectives. They work collaboratively with the 340B program, physicians, nurses, care coordinators, and associated business partners to improve medication access and ultimately patient outcomes. This position will play a role in patient access, care coordination, and service line implementation. This roll will act as a primary resource supporting HCO services by collaborating with stakeholders to efficiently develop and deliver recommended solutions; following HCO service processes; finding solutions to HCO service challenges; ensuring implementation and on-boarding tasks are executed in order to achieve HCO initiatives; identifying and communicating status and adjustments to HCO milestones and transitions between project phases; and executing system changes as they relate to HCO services. This role is designed for an individual who excels in overseeing complex projects from conception to completion. The successful candidate will lead special projects that drive strategic initiatives and enhance operational efficiency.

General

  • Supervise the logistics required to ensure HCO operations are successful
  • Monitor project progress, adjusting as necessary to ensure successful completion.
  • Develop, coordinate, and execute special projects from conception to completion.
  • Work closely with cross-functional teams, specifically the 340B team, and pharmacy team to ensure HCO services are operating effectively.
  • Identify potential service risks and challenges and develop strategies to mitigate them.
  • Provide solutions to problems that arise within the HCO services.
  • Serve as a primary resource to train, and onboard subsequent team members
  • Assist Executive Director of Health Transformation with new HCO initiatives

 

Patient Care

  • Support HCO service lines and facilitate HCO patient encounters
  • Utilize telehealth technology to coordinate with HCO services
  • Facilitate HCO and DTE on-boarding and implementation schedules 
  • Executed initiatives designed to expand community based and mobile healthcare services to increase access

 

Pharmacy Services

  • Manage HCO service formularies
  • Analyze claims data to identify expanded access opportunities
  • Support non-HCO dedicated pharmacy staff assisting with HCO services

 

Care Coordination

  • Coordinate HCO member on-boarding and implementation schedules
  • Schedule HCO patients with HCO Pharmacists and Provides
  • Monitor HCO and related service patient adherence
  • Assist HCO contract pharmacy distribution logistics are uninterrupted
  • Coordinate live HCO events

 

Service Development

  • Assist contract pharmacies with HCO service implementation, sustained operations, and appropriate application of service line specific operational parameters 
  • Act as the front-line HCO patient liaison to address challenges or answer service-related questions
  • Understand core elements related to purchasing, compliance, operations, and logistics of HCO services

Qualifications

Education

  • Associate or bachelor’s degree in business administration, project management, or a related field | Preferred
  • High school diploma or GED | Required

 

Experience

  • 2 years minimum experience in related healthcare settings | Required
  • 3 years’ experience in ambulatory care, transitions of care, or equivalent experience | Preferred
  • 2 years of experience in health system supervision, project coordination or management | Preferred
  • Proven ability to manage multiple projects simultaneously | Preferred
  • History of creative problem solving in dynamic service environments | Preferred
  • Strong organizational, problem-solving, and analytical skills | Preferred
  • Excellent communication and interpersonal abilities |Preferred

 

Licensure/Certifications

  • Ability to obtain licensure as a pharmacist or Pharmacy Technician by the Arkansas State Board of Pharmacy | Required
  • Apexus-340B University | Preferred
  • Apexus-Certified 340B Expert | Preferred

 

Work Schedule

  • Weekdays, evenings, weekends, early morning, on-site, off-site, and holidays as needed
