Overview

The Clinical Pharmacy Coordinator is responsible for coordinating clinical pharmacy services and ensuring that the pharmacy provides optimal clinical services on a daily basis. The Clinical Pharmacy Coordinator facilitates the development, implementation, and maintenance of systems that promote desirable patient outcomes through safe, appropriate, cost-effective medication therapies.

Responsibilities

  • Ensures that the pharmacy meets or exceeds all CMS (Medicare), Arkansas Department of Health, and Arkansas Board of Pharmacy standards related to pharmacy
  • Develops and maintains a medical staff-approved formulary
  • Reviews the performance and documentation of clinical activities of all pharmacists, including documentation of the financial impact of clinical activities
  • Prepares reports on pharmaceutical care and pharmacoeconomic activities including financial impact, as directed
  • Recommends resources needed for the department
  • Participates in pharmacy and therapeutics committee activities
  • Plans educational programs based on needs identified from quality assessment activities
  • Reviews and evaluates medical literature to support pharmaceutical care and pharmacoeconomic programs
  • Interprets medical information and applies it to resolve problems or answer questions
  • Collects and analyzes microbiology data and produces an antibiogram at least annually
  • Participates in community educational programs (e.g., diabetes classes, health fairs)
  • Monitors drug therapy regimens for contraindications, therapeutic duplications, drug-drug interactions, drug-food interactions, drug-disease interactions, drug-laboratory interactions, allergies, and appropriateness of drug and dose
  • Recommends evidence-based medication therapy regimens and monitoring plans
  • Provides clinical consultation and clarification to practitioners
  • Documents all clinical activities and interventions accurately and completely
  • Develops and implements pharmacokinetics consult protocols
  • Facilitates the development and implementation of evidence-based treatment guidelines, protocols, formulary changes, and critical pathways as approved by the appropriate committees
  • Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed
  • Recommends conversion of intravenous to oral therapy when appropriate
  • Develops criteria for medication-use reviews
  • Participates in medication-use review activities
  • Develops policies and procedures that promote safe medication-use practices
  • Provides medication-use safety education to healthcare providers and other appropriate staff
  • Participates in quality control and performance improvement activities

 

WORK SCHEDULE

Maximum of 80 hours per 2 week period, but hours may be long and irregular; includes evening, weekends and holidays as necessary.

Qualifications

REQUIRED:

Graduate of an ACPE accredited School of Pharmacy with a Doctor of Pharmacy degree. Degree must be provided.

Currently licensed to practice pharmacy in the state of Arkansas.

 

Preferred:

Board Certified Pharmacotherapy Specialist or Post Graduate Year 1 Residency

5 years' experience in a health sustems pharmacy practice settign with clinical pharmacy supervision experience.
