RN - Intake Coordinator - Home Care/Hospice

Assesses appropriateness of referrals to provide skilled care to home health patients on an intermittent basis, and Hospice patients per regulations. 

Interact with referral sources and agency staff to assure smooth, efficient transition on to services. 

Assists in all areas of Home Care and Hospice as needed, within the employees scope of practice and experience.

Demonstrates the ability to coordinate effective transition of the patient from referral setting on to Home Care/ Hospice services.


Coordinates and arranges for patients needed home equipment, Hospice medications, infusion therapy and/ or home nutritional needs.

Demonstrates the ability to manage multiple phone calls and patient situations without compromising the care coordination of any of the different situations being handled.

Accepts responsibility for coordinating the care transition of patients by interacting with referral sources, physicians, patients, families and staff members.


Develops and maintains effective communications mechanisms with staff members and case managers/ DC Planners/ social workers of the hospitals, physicians, families and the community.


Keeps Clinical Supervisor aware of pending referrals.


Performs informational pre-admission visits to hospital patients/ families regarding home care/ Hospice services to assess needs and identify services.

Keeps current regarding home care and hospice rules and regulations, eligibility for services, requirements and reimbursement criteria.

Participates in Interdisciplinary Team meetings and Regular Staff Meetings, updating professional staff regarding regulatory and coverage issues affecting patient care/ eligibility.

Assists in interpreting federal and state governmental regulations for home care/ hospice for all levels of personnel, including referral sources and patients/ families as needed.
Demonstrates effective communication methods and skills using the lines of authority appropriately.

Dresses appropriately and meets hygiene standards in accordance with NARMC Dress code.

Demonstrates professional medical care to the sick and disabled without regard to race, sex, age, creed, religion or inability to pay.

Has knowledge and demonstrates appropriate care for patients’ growth and developmental needs.

Complies with NARMC HIPAA Privacy and Security policies.

Accurate assessment of patient care needs and eligibility for services.

Ensures the patient record documentation reflects appropriately the requested services and orders obtained from referral sources/ physicians.

Maintains a professional standard of nursing in the office, the hospital and in the community that reflects the philosophy and professional standards of the department and regulatory bodies.

Demonstrates awareness of cost controls and strives to reduce department costs.

Supports hospital and departmental policies.

Maintains strict patient confidentiality.

Assists with orientation of new employees as requested.

Performs supervisory visits of home health aides as needed to assist with Agency compliance.


Performs nursing and informational visits to the patient homes on an as needed basis.


Assists with On Call and/ or PRN visits on an as needed basis.

Submits required paperwork accurately and timely in accordance with Agency policy and Federal and State regulations.

Demonstrates accurate/efficient utilization of equipment/supplies.

Prioritizes and utilizes time efficiently.

Demonstrates flexibility to departmental demands and scheduling.

Sound, effective decision making in all aspects of job performance.

Develops and maintains effective communications mechanisms with staff members and other departments of the hospital, physicians, families and the community.

Demonstrates awareness of responsibility and accountability for own actions and assigned responsibilities.

Reviews departmental policy & procedures yearly.

Understands responsibility to report issues with privacy, fraud and abuse, compliance and ethical conduct.

Participates in Departmental Performance Improvement and Quality Assurance activities.

Performs/ reviews record reviews on active and/ or discharged patient charts and reports results.

Communicates at appropriate comprehension level to patients/families.

Reports directly to the Clinical Supervisors and ultimately to the Director of Home Care all lapses in patient care and other employee issues.

Performs job duties as a team member to assure the effectiveness of, and harmony within, the department.


Assists with public relations/ community outreach and activities as requested.


Demonstrates a commitment to the Mission, Vision, Values and Standards of behavior of NARMC.

Creates a positive image of NARMC at all times.


Adjusts personal schedule to complete work as appropriate.

Performs other duties as assigned.







Graduate from an approved school of nursing.


Current RN license from the Arkansas State Board of Nursing



Must have at least two or more years’ experience in nursing.


Computer literate and demonstrates ability to be organized

Shall have at least one year of home care and/ or Hospice experience.


Degrees, Licensure, and/or Certification:

Arkansas RN, AHA BLS-CPR


BLS certification required.


Must have reliable transportation and a valid Arkansas driver’s license.


Knowledge, Skills, and Abilities:

Must be able to assess, evaluate and communicate skillfully and effectively.


Must demonstrate sound knowledge and actions in patient care and decision making


Participates in continuing education to broaden knowledge and acquire skills necessary to be effective in all aspects of job performance


Demonstrates a desire for self-improvement


Ability to read, write, speak and understand English


Shall have experience in geriatric care
