Overview

Responsible for the coordination and assistance of the administrative functions for population health which include, Patient Center Medical Home (PCMH), Collaborative Accountable Care (CAC), Accountable Care Organization (ACO), Meaningful Use (MU), and Physician Quality Reporting System (PQRS).

Responsibilities

  • Effective communication with all staff members
  • Work with Clinic Coordinators to improve work flow process to ensure scheduling of high priority patients
  • Promotes clear communication amongst a care team by ensuring awareness regarding patient care plans.
  • Patient outreach including communication to promote preventative health to ensure the quality care of our patients
  • Collaborates with Clinic Administration and Clinic Staff to enhance patient flow, patient care resulting in continuous achievement of operational and patient care objectives
  • Ensures compliance with hospital and regulatory advisories and mandates
  • Improves customer experience with modeling and reform
  • Implements Policy and Procedures

Qualifications

Education:

 

High School or GED. Prefer CCMA.

 

Experience:

 

3 plus years of clinic supervisory or management experience

 

Degrees, Licensure, and/or Certification:

 

Knowledge, Skills, and Abilities:

 

Ability to read, write, speak and collaborate in English.

Must have excellent organizational and administrative skills, customer service skills

Ability to work collaboratively with all team members and external resources and customers

Must provide outstanding customer service

Must be able to read, write, speak and understand English
